Check-In
Patient Registration
When you come to Rex Healthcare, a receptionist will ask you for your name and the type of procedure(s) for which you are being seen. You will then be directed to the nearest registration area and will meet with a registration specialist. If you have not been pre-registered, we will ask you for detailed information at this time.
The information you provide at registration will:
- Ensure you have consented to the services as ordered by your physician.
- Ensure your physician and other caregivers have the information they need to provide your care without delays.
- Ensure we have your insurance information to file your insurance claims for you.
On the day of your procedure, please sign in at the registration desk and be seated in the waiting area. If a patient is a minor or is unable to sign the surgical consent form, the parent, legal guardian or person with power of attorney must be present to sign the consent and show proof of their legal designation. When called to the procedure preparation area, your nurse will explain your plan of care and what you should expect while at Rex.
Even if you have been pre-registered, you need to check in with registration before your procedure or treatment. In order to perform your scheduled procedure, you must:
- Arrive on time
- Adhere to food and drink guidelines
- Bring someone to safely drive you home
- Have not had an unexpected change in your medical condition
Where you go to check-in depends on the type of procedure or treatment that you will receive.
